On December 29, 2025, the Centers for Medicare and Medicaid Services (CMS) announced the first year of funding for the Rural Health Transformation (RHT) Program. The RHT Program will provide $50 billion over five years, about $147 million to $281 million per state, to strengthen and modernize health care in rural communities. Each state developed unique plans to expand access, strengthen workforce, build infrastructure and technology, and support care delivery innovation.

As described in Public Law 119-21 and outlined in the Notice of Funding Opportunity:

  • 50% of the funding is distributed equally among all approved states.
  • 50% of the funding is allocated based on a variety of factors. Factors include individual state metrics around rurality and a state’s rural health system, current or proposed state policy actions that enhance access and quality of care in rural communities, and application initiatives or activities that reflect the greatest potential for, and scale of, impact on the health of rural communities.
