Goals and Accomplishments

The Minnesota team intended to define family engagement.

The Minnesota team identified ways to improve family engagement at the systems level and built a strong collaborative team.

The Family to Family Health Information Center in Florida is Family Voices of Minnesota.

The team is pleased with:

  • The team’s level of communication

  • The team’s commitment to improving family engagement and involvement

  • Connecting with members of our team and other teams

  • Learning about the work of other teams

  • Better understanding the Family Engagement in Systems Activation Tool, trying the tool and planning to use it in the future

  • The work around defining family engagement

The team hopes to:

  • Implement Gillette Family Engagement in Clinical Initiatives next

  • Meet with Minnesota Department of Health’s Children and Youth with Special Health Care Needs (CYSHCN) Section leadership to talk about working together to improve family engagement

  • Expand family engagement to other state systems

  • Discuss how they might collaborate with committees with similar goals such as the Maternal and Child Health Task Force subcommittee

Advice for future cohorts:

  • Have a clear understanding of the overall purpose and goals of the Learning Collaborative. Then determine if your team’s project goals align with those of the Learning Collaborative.

  • Be narrow in the scope of what you want to accomplish.

  • Choose a project that aligns with another current project
