Health care transition, or HCT, is about moving to an adult model of care where you are the main person making decisions about your health. For many youth and young adults, this can be a challenging process. In our new virtual world full of telehealth doctor appointments, how can we make the transition to adult health care smoother? Having a joint telehealth visit could be a good place to start, especially if you have a chronic condition. This is a virtual meet-and-greet appointment that can help prepare you for your transition to a new adult doctor.

This webinar from Got Transition, CT KASA, and Family Voices highlights a new video and social story that can help you learn about how to engage in tele-transition services to help you transition from a pediatric provider to an adult provider.
